APFT Failure Counseling
On ____________, you failed the semi-annual Physical Fitness Test. IAW AR 350-15, Chapter 1, you are required to take and pass the APFT twice each year. You are being counseled so you may clearly understand the serious actions which may be initiated against you for failing to achieve a passing score on each of the three APFT events. _______________Effective immediately you are considered a PT failure. You are informed that Report for Suspension of Favorable Personnel Actions has been completed. Your personnel records are now flagged and you are ineligible for promotion and reenlistment. Your records will continue to be flagged until you pass the APFT. All PT failures will be given a "FOR THE RECORD" test every 90 days until successfully completed. Your records will be flagged until you pass the APFT. In addition to attending regularly scheduled PT formations, you will attend REMEDIAL formations at ____________________. Continued unsatisfactory conduct/performance cannot and will not be allowed to continue. Should you continue to regularly fail the APFT without any medical justification, you could be processed for elimination from the service under Chapter 13 of AR 635-200. If conduct worsens, you could be discharged for misconduct under Chapter 14, AR 635-200, resulting in an other than honorable discharge. If separated for your duty performance, the least favorable character of discharge you could receive is a general discharge. If separated for misconduct, you could receive an under other than honorable conditions discharge. Both may cause undue hardships in civilian life and may reflect on your eligibility for veteran's benefits, eligibility for reentry into the Armed Forces, and acceptability for employment in the civilian community.
